This is the Likeness Release template I use for my video, film and photography productions.

I highlighted fields that should be filled by you and conditions that you can adjust in green color.

I highlighted the part your on-cam talent should fill in yellow color.

You will fill your name, project name and shoot date. You should also double check the timeframe of the agreement, highlighted green. Currently it is ‘in perpetuity’ but you may want to change that based on your agreement.
